Sam has a meeting at school about Alex's problems. Gallant's sister is due to leave. A chronically ill teen gets bad news after being in a car accident, and an end stage AIDS patient is surprised by the standard of care she's receiving.

Carter's back.

- Luka tells Kerry he's going back to Africa in January and asks her to inform administration about his resignation. She tells him to put it in writing and that County needs him as much as the Congo.

- Gallant and Pratt bicker regarding Pratt's interest in Valerie.

Carter blasts in, sees Kem on an exam table with a doctor performing an ultrasound. We can hear a robust heartbeat. Carter says Kem? Kem say Look. Carter looks at the screen. A fetus, moving. Kem says It's your son.

Pratt injures a patient while doing a procedure Carter told him not to do. He breaks the boy's neck.

The doctor says Normal extremeties, good fetal movement. Carter says Any neural tube defects? The doctor says No. Looks great. Kem says I couldn't help it. I wanted to know. He's beautiful isn't he. Carter says A son.

Luka speaks some Spanish.

Sam is in the middle of a trauma with Pratt when Frank tells her the Principal at Alex's school wants her there asap. She says great. No, Sam, trust me, you won't like this one!

At the school, Sam talks to Principal. She says she doesn't understand why he called her, she thinks Alex cut himself on the playground and the school nurse gave him a stich - our Sam cracks that her colleague did a lousy job!

But, surprise! Alex stiched himself and freaked out a few kids in the process. The boy refuses eye contact with his mother, specially when the Principal gets a County suture kit from his desk drawer and asks if Sam knows where Alex got it. She does, Mr. Principal, she does! Actually Sam says she works at County, but she doens't know how Alex got it and she will talk to him, it will never happen again.

Uh uh, Sam, you should watch Boston Public! Principals always have more to say! The Principal tells Sam that talking to Alex and making sure it won't happen again would be good, but bringing a needle to school violates their no gun policy. I don't know about you, kids, but I miss the good old days when schools didn't need a no gun policy!

The Principal asks Alex to wait outside, he wants to talk to Sam alone. Alex is worried, wouldn't you? The Principal says Alex is suspended for three days, Sam thinks it is fair. The Principal asks Sam if he may suggest something, she says yes. He tells her that it is not uncommon to see acting like Alex's in households without a positive male role model... Sam interrupts, ok, hold it. The Principal continues on, he suggests that during Alex's time off Sam could arrange for the boy to see someone. Sam asks if he is saying her kid needs a shrink and the Principal replies that Alex is young enough that with help he can be turned around.

Sam is not happy! She says her son is not crazy, he is just a kid who does stupid kids things sometimes, what didn't the Principal ever do soemthing stupid? Looks like the answer is no, no and no, because the Principal tells Sam she needs to have Alex evaluated!

Sam, honey, forget about the shrink, get Luka! Your son will have a positive male role model... and you get the male part all for yourself!

Carter is treating Larry, He and his 19 years old son Peter were in a car when they crashed agaisnt a phone pole. Paramedics think Larry may have had a syncopal episode and he is the worst of the two. Carter takes the father, Pratt the son.

Luka enter the trauma room and is surprised to see Carter there, says you are back. Carter says a week ago, been holed up at the house. Luka asks Kem, right, and while treating Larry our Papa Carter says Kem was the one doing the HIV trials in Kinshasa and asks if Luka ever met her. Luka tells Carter he hasn't, must have gotten past him.
